This product does NOT contain the American Medical Association (AMA)'s CPT-4 codes.This list of the Alpha-Numeric HCPCS contains the 5-character alpha-numeric procedure code, modifier codes, and descriptions (long and short) of procedures and modifiers, along with a variety of claims processing, coverage, and other HCFA Administrative data related to each procedure code. These codes are used by the Medicare program and by many of the Medicaid State agencies on claims. Because of the proprietary nature of the AMA's CPT-4 codes, HCFA is prohibited from releasing those codes to other than its claim processing contractors.
