Presents the historical perspective and rationale for the new technique of metal-on-metal hip resurfacing for total hip arthroplasty, in light of results for the authors first 500 cases. Demonstrates a detailed step-by-step technique for reliably accomplishing this procedure through a minimally invasive, four-inch posterior ap­proach. This bone-conservative procedure is technically more difficult than stemmed total hip arthroplasty and should only be attempted by experienced hip surgeons.
